Gateway Plaza and as well as offsite clinics

KEY RESPONSIBILITIES

  • Provide day-to-day leadership, guidance, and support to the Community Health Worker (CHW) team and departmental managers.
  • Oversee daily operations, workflow, and service delivery to ensure program goals and client needs are met.
  • Foster a positive and supportive team culture through strong leadership, emotional intelligence, empathy, and trust-building.
  • Communicate clear expectations, provide ongoing coaching and feedback, and support staff performance and development.
  • Train, mentor, and onboard Community Health Workers, including developing training materials and continuing education opportunities.
  • Promote ongoing professional development and provide emotional support to team members.
  • Serve as a liaison between Community Health Workers, clients, healthcare providers, community organizations, and other stakeholders.
  • Coordinate with healthcare providers and community partners to ensure seamless and effective service delivery.
  • Facilitate community outreach initiatives and strengthen community engagement efforts.
  • Monitor program outcomes through data collection, analysis, reporting, and performance tracking.
  • Identify opportunities for process improvement and support implementation of best practices.
  • Demonstrate cultural competence and effectively engage with diverse populations and communities.
  • Support departmental and organizational initiatives as needed.
  • Perform other duties as assigned.

REQUIREMENTS

  • High School Diploma/GED is required. Associate's Degree is preferred.
  • Requires 7-10 years of relevant experience.
  • Previous leadership experience is preferred.
  • Requires knowledge of applicable products and markets.
  • Requires working proficiency in MS Office (Word, Excel, PowerPoint).
  • Must be familiar with the software used to connect with customers and gather their information.
