Job Overview:

Partnering with Primary Care Physicians in the Deaconess network to provide Primary Care and Acute services for residents at Long Term Care facilities.

Preferred Certifications/Education/Experience:

Experience in Long Term Care or Geriatrics

Required Certifications/Education/Experience:

Board Certified Nurse Practitioner or Physician Assistant with license to practice in Indiana.
