Job Overview:
The Diabetes Clinician will work directly with the patients to prepare them for self-management after discharge when seen inpatient or will provide outpatient diabetes education. The Diabetes Clinician will assess outpatients to determine if they are appropriate for individual education or group education sessions. Outpatient responsibilities also include a monthly diabetes support group meeting. Other project to improve diabetes care at Deaconess at both the patient-level and organization-level. The Inpatient Diabetes Clinician will use their nursing skills to education patients who are new to diabetes or insulin or need assistance in monitoring their care. Nurses in this role will work directly with other healthcare providers to improve individual patient's diabetes management, maintain hospital diabetes references, improve diabetes results/outcomes through various projects.
Diabetes Clinician position which will provide both inpatient diabetes education and outpatient diabetes education (individual patient appointments, monthly group diabetes education classes, and monthly diabetes support group meeting) at Deaconess Memorial Medical Center in Jasper, Indiana. Hiring for 0.8 FTE to work 4 weekdays every week.
Required: Certifications/Licenses/Education
- Graduate from a nationally accredited school of professional nursing
- Must have a Registered Nurse (RN) license that allows employee to practice in Indiana.
- CDCES (Certified Diabetes Care and Education Specialist) certification, preferred but not required
