100% remote with in-office requirements as needed for training and meetings.

What You’ll Do

As a Clinical Pharmacist Specialist, you’ll play a key role in delivering comprehensive, value-based clinical care. You’ll have the opportunity to:

• Meet with patients individually (via telehealth) to provide drug and disease state education that improves health outcomes and reduces costs
• Partner with patients to overcome medication adherence challenges
• Formulate and communicate clinical recommendations and visit summaries with healthcare providers
• Proactively identify and address barriers to effective care delivered through the PBM
• Collaborate with internal departments to leverage technology and health information for better outcomes
• Provide medication therapy management (MTM) services to members
• Deliver clinical reporting and insights to clients, alongside account management teams
• Respond to requests from True Rx staff, clients, and members regarding drug therapy, formulary, and clinical program topics
• Educate clients on True Rx clinical programs to drive engagement and adoption
• Support the Clinical Innovation Pharmacist in implementing new and enhanced clinical initiatives

What We’re Looking For

• PharmD or BS in Pharmacy (PGY1 residency preferred)
• Active pharmacist license
• Board certification in ambulatory care or significant experience as a clinical pharmacist in ambulatory settings (highly preferred)
• Strong familiarity with treatment standards for metabolic conditions, including diabetes and cardiovascular risk reduction
• Excellent communication skills with patients, clients, and healthcare providers
• Experience with motivational interviewing and patient education
• Ability to work independently with minimal supervision and thrive in a dynamic, team-oriented environment
• Proficiency in Microsoft Word, Excel, and Outlook

Preferred: Experience in PBM settings and familiarity with P&T strategies, prior authorization processes, and clinical program trends.

• Occasional Trailblazer: Travel occasionally as work requires.
