As a Clinical Care Coordinator, you play a key role in connecting members to the clinical support they need to maximize health and wellness. This role is ideal for someone who thrives in a fast‑paced environment, enjoys working with details, and takes pride in delivering excellent service. Your work directly supports patients, pharmacists, and clinical programs that improve outcomes and experiences.
Work in office: 40% ; Work from home: 60%
What You’ll Do:
Care Coordination: Coordinate contacting, enrolling, and scheduling patients for clinical services while ensuring a smooth and supportive experience.
Patient Information Management: Gather, review, and maintain accurate patient information, including medication history and biometric data.
Communication & Triage: Handle incoming calls and communications from patients, prescribers, and partners, triaging needs appropriately.
Clinical Program Support: Assist with clinical program communications, documentation, data tracking, and ongoing process improvements.
Collaboration: Partner with pharmacists, internal teams, and external partners to resolve patient concerns and support clinical initiatives.
Additional Responsibilities: Support inventory coordination, reporting, research requests, and other projects as assigned.

Strong Communication Skills: Clear, professional written and verbal communication skills.
Organization & Attention to Detail: Ability to manage multiple priorities while maintaining accuracy and organization.
Adaptability & Teamwork: A willingness to learn, adapt to new processes, and collaborate effectively with a team.
Education & Experience: High school diploma with some college preferred; healthcare, pharmacy, or public health experience is a plus. Familiarity with HIPAA and data entry preferred.
